The Information We Collect and How We Use the Information
Registration Form
In order to participate in the Prevention in the Care Setting workshop online visitors must fill out a registration form. This form asks for information such as your name, address, title, training program or affiliation, training program director and e-mail address. This information is collected so that we can contact you about the workshop and so that we have a record of who has taken the course.

The registration form also collects information on your work setting. This information is collected for internal use so that we can track what types of physicians and healthcare workers are using our online education programs.

The information collected on the registration form will not be shared or sold to any individual or organization.

Post-Test Results
Post-Test results are accessible only to the person who takes the test and HIVMA and IDSA staff. The result for each question, without any personally identifiable information attached, is used to calculate the normative data so that users can see how they have done on the tests in comparison to others who have taken the test.

Test results without personally identifiable information attached will be used by HIVMA and IDSA to evaluate the effectiveness of the course.

Program Evaluation Form
Users are required to fill out a program evaluation before printing out the Certificate of Completion. The form collects information about the user's experience with the program. The information collected on this form will be used internally to evaluate the effectiveness of the course. HIVMA will not store personally identifiable information in association with the program evaluation.

Aggregate Log File Data
Log data is information automatically collected from the computers of site visitors. It can be viewed only in aggregate and does not include any information that is personally identifiable. Log data includes such things as the type of computer operating systems, the types of Internet browsers, and the name of the Internet Service Provider used by visitors to the site. Log data also includes quantitative information on which site pages and activities are visited most often. This information may be used to aid future site development by showing which features and information users access most.

Security
HIVMA maintains an information environment that protects the security, privacy and integrity of all information collected and used within the site. Information is stored within server facilities that use the latest industry standards and practices for protecting information.
